Appropriate reasons for referral to the diabetes clinics include, type 1 diabetes, poor glycaemic control in patients on maximum oral hypoglycaemic medication who require insulin, presence of proteinuria and renal impairment, painful neuropathy poorly responsive to standard therapy and uncertain type of diabetes.

- with pure hypercholesterolaemia with cholesterol >7.5mmol/L and/or LDL >4.9mmol/L
- and a Dutch Lipid Clinic score of at least five
- and/or who meet the Simon Broome criteria
Please state clearly how the patient meets criteria for referral
Enclose the results of:
- two full lipid profiles (cholesterol, HDL, LDL, triglycerides), ideally pre-treatment

Hypertriglyceridaemia

Consider referral in patients with:
- fasting triglycerides persistently >10mmol/L (if alcohol and/or poor glycaemic control excluded)
- fasting triglycerides persistently 4.4 - 9.9mmol/L and nonHDL cholesterol >7.5 mmol/L
Please state clearly how the patient meets the criteria for referral. Enclose the results of:
- two fasting lipid profiles
- renal and liver profiles
- HbA1c
- TFTs
Consideration for PCSK9 inhibitor therapy

Consider referral in patients:
LDL is persistently >5mmol/L without CVD or >3.5mmol/L with CVD
LDL is persistently >4mmol/L if at high risk of CVD* &>3.5 mmol/L if at very high risk*
PCSK9 inhibitors are currently not recommended for primary prevention
Please state clearly how the patient meets the criteria for referral
Enclose the results of:
- two full lipid profiles (cholesterol, HDL, LDL, triglycerides) at least three months apart
- renal and liver profiles
- HbA1c
- TFTs
Statin intolerance
Attempt trial of two or three statins, see Stain Intolerance [docx] 110KB
Consider referral of high-risk patients in whom treatment goals are not attained
Please state clearly how the patient meets criteria for referral and include details of all statins prescribed (with dates and doses) and any relevant laboratory results (e.g CK)
