Our services are based in GP surgeries, community health centres and hospitals across Tower Hamlets. We also see patients in their homes.
We will work with you to create a care package that fits your needs and our staff are skilled in treating a wide range of conditions and issues.
Referrers will use one single form for all of the community health services managed by Barts Health and East London Foundation Trust. We anticipate this will be a simpler and more efficient process for you.

We receive referrals from SEN coordinators (SENCOs), doctors, nurses, therapists, child and adolescent mental health professionals, teachers, educational psychologists and social workers.

If you would like to refer a child or young person to be seen, please fill out the SPA referral form.
If you are making an ASDAS referral, please fill out both SPA Referral form and the ASDAS referral form.

Do note: The family needs to be residing in Tower Hamlets and registered in a Tower Hamlets GP Surgery to be accepted.
We are not an emergency service – we run an outpatient, community service.
